How does the Refer A Friend special Work?
- For each and every new patient you refer who starts their weight loss program by March 31, 2010, you will receive one FREE weekly visit on your subsequent office visit.
This offer applies only to our weight loss program and not to facial cosmetic treatments.
Are there any limits to the number of Free weekly visits I can receive?
- There are no limits to the number of Free weekly visits you can receive. You may refer as many patients as you like and you will receive a FREE weekly visit for each and every new patient who starts their weight loss program by 3/31/10.
When must a new referral ‘Start’ their program?
- To receive a Free weekly visit, a new patient referral must have been seen & evaluated by one of our physicians and started on their diet by 3/31/10. A new patient having ‘made an appointment’ by 3/31/10 does not constitute ‘starting’ the program.
When can I receive my FREE weekly visit(s)?
- You can apply your FREE weekly visit(s) on any subsequent visit through 2011.
How are referrals documented?
- All new patients will be asked and must declare and document on their initial visit the one name of the person who referred them to our office. We will give a FREE weekly visit only to the person who was indicated to us by the new patient, provided the new patient begins their program by 3/31/10. If you refer a patient but this new patient does not indicate that you were the one who referred them, then unfortunately you will not be able to receive the FREE weekly visit.
Can I redeem my FREE weekly visit(s) for cash value?
- No, visits cannot be redeemed for cash.
Can I transfer my Free weekly visits to my friends or family?
Unfortunately no. The FREE weekly visits are non-transferable and therefore only redeemable by you at your subsequent office visit.
I am a former patient. Can I also refer and receive a subsequent free weekly credit?
- Yes. The free weekly visit will apply to the next office visit on which you start your diet. It will not include the cost of any routine Lab or EKG examination that you may require.
I am not yet a patient. Can I refer a friend and receive a FREE weekly visit?
- Yes, but the free weekly visit cannot be redeemed on your ‘Initial’ office visit. It can be applied to any subsequent visit. Additionally, two new patients cannot refer each other and each receive Free weekly visits; only one will be able to receive the free weekly visit. In the event that two new patients claim to have referred each other, only the first person who starts our program and declares the referral will be eligible to receive the Free weekly visit.
What is included in the FREE visit?
- A FREE Weekly visit consists one FREE office visit and one week (7 days) of any medication and/or supplements that you have been currently taking.
What is not included in a FREE weekly visit?
- Not included are the costs of:
- Any Lab or EKG examiniations that may be routinely required,- Any liquid protein shakes,- Other limitations may apply.Can I refer a family member?
- Yes. You can receive a Free weekly visit for any new patient who starts their program by 3/31/10.
